A PREVALENCE STUDY OF PULMONARY HYPERTENSION IN CKD PATIENTS ON MAINTENANCE HEMODIALYSIS

Objective: Chronic kidney disease (CKD)-is an emerging public health problem. Cardiovascular disease is the most common cause of mortality in CKD. Pulmonary hypertension (PH) is an overlooked cardiovascular complication of CKD, especially in end-stage renal disease (ESRD). PH is defined as "a mean pulmonary artery pressure more than or equal to 25 mmHg at rest or 30 mmHg at exercise. This analytical cross- Methods: sectional study was conducted in Department of Nephrology, Government Mohan Kumaramangalam Medical College and Hospital from April 2023 to March 2024. 108 patients of end stage renal disease (ESRD), on maintenance hemodialysis (HD); underwent Trans thoracic Echocardiography were selected. Pulmonary hypertension was further divided into mild (PAP b/w 30-45mmHg), moderate (PAP b/w 45- 65mmHg) and severe pulmonary hypertension (PAP > 65mmHg). The effect of different vascular accesses, age, gender, dialysis vintage on the development of pulmonary hypertension was observed. Out of 108 patients, 50 patients (54%) had pulmo Results: nary hypertension (PH); 30(60%) had moderate, 15(30%) had mild, and 5 (10%) patients had severe pulmonary hypertension (PH). Pulmonary hypertension was present in 45(50%) patients with AVF. Pulmonary hypertension was more common in females; present in 28 females (56%) and 22 males (44%), that was statistically significant (p<0.05). Mean duration of hemodialysis in (months) of patients with PH was 36 ± 8 vs. 12 ±7 in patients without PH (p<0.05). Age had no relation to development of PH. This study found a high prevalence (54%) of PH Conclusion: among dialysis patients. Statistically significant association was observed between presence of malnourished patient, moderate LV dysfunction, presence of concentric LVH and Pulmonary hypertension.
