Off label use of rituximab at a tertiary neurology centre

Rituximab (RTX) is approved for use at Imperial College Healthcare NHS Trust (ICHNT) via the Advanced Immunotherapies MDT (AIM) for a range of neurological conditions with a record of expected outcomes of treatment required. Aim to assess the adherence to local policies and NHSE commissioning criteria for the use of RTX.
Methods
Retrospective audit of subjects between April 22 & June 23 for commissioned indications who had approved funding applications for RTX.
Results
Indication was documented for 100% of patients, 53% had documentation of intended treatment duration 86% had a follow up arranged. 51 doses of RTX were issued for 37 patients for 7 indications (15 Myasthenia Gravis, 11 neuropathies , 4 myositis , 3 Autoimmune Encephalitis , 2 sarcoid ,1 Transverse Myelitis,1 CNS vasculitis). 25 were for indications not commissioned by NHSE; 23 were commissioned but did not have a Blueteq submission and Blueteq funding submissions were completed for 3. For the 23 indications without submissions there was not enough documentation to determine eligibility. Total expenditure on RTX was £26K, £14.5K of this was for indications funded by NHSE.
Conclusion
AIM meeting allows a structured approach to giving RTX. A significant number of indications are now commissioned by NHSE
