Analysis of novel five question tool for identifying secondary headache in the neurology emergency-care service

Headache presents frequently to acute-care and neurology services, where evidently discriminating between primary headaches and secondary headaches is paramount. A recent study proposed a five- question clinical tool looking at the following key components thunderclap-onset, neurological signs and symptoms, systemic features, and age greater than 50. The study indicated reasonable sensitivity and specificity in 79 patients, but the need for further assessment in a larger sample group. The neuro-SDEC (same-day emergency care) service is a consultant-led neurology service within the acute and emergency care unit at UCLH. It investigates over 100 patients presenting acutely per month. A retrospective review of patients presenting to SDEC with a headache over a several-month period is being performed. This aims to review the variety of headache diagnoses presenting acutely to the neuro-SDEC and subdivide them into primary and secondary headaches. Headaches will be classified by symptomatology, including common features seen in secondary headaches. The study also aims to further assess the five-question tool in a larger data pool. Results of the study will be presented in the poster.
