Characteristics of asthma in patients with NSAID hypersensitivity

Introduction: NSAID hypersensitivity (NH) is one of the commonly encountered drug hypersensitivity reactions. The broad clinical spectrum and the presence of multiple underlying mechanisms make the diagnosis of NH challenging. EAACI has identified 5 clinical phenotypes to facilitate the diagnosis and understanding of NH. Determining the relation between phenotypes and various comorbidities will provide guidance for establishing appropriate diagnosis and treatment. In our study, we aimed to elucidate the clinical characteristics of asthma in patients with NH and to examine its relationship with phenotypes of NH. Methotds:Patients who presented to our clinic, between January 1, 2019, and December 31, 2022, and were diagnosed of NH were retrospectively reviewed. Age, gender, presence of asthma, atopy and N-ERD, eosinophil counts and clinical phenotype of NH of the patients were were recorded. Patients were categorized into Group-1 (with asthma) and Group-2 (without asthma). Results: A total of 291 patients were included in the study (67% female, mean 44 years old). 124 patients (42.6%) had asthma. Group-1 and Group-2 had similar characteristics in terms of age and gender distribution. The mean eosinophil count was 460 cells/mcL in Group-1 and 190 cells/mcL in Group-2 (p<0,001) ,atopy was observed more frequently in Group-1(p<0.05). In Group-1,NERD phenotype (77.4%) was dominant, and in Group-2, NSAID-induced urticaria/angioedema (NIUA) (37.1%) Conclusion: When asthma accompanied NH, eosinophilia and atopy were observed more frequently. Moreover, the presence of unclassified patients in the new phenotyping indicates the inadequacy and potential for development in the classification of NH phenotypes.
