Overlap of treatable traits in asthma and COPD in Dutch primary care

Background: The Dutch Asthma/COPD service supports general practitioners by diagnosing and monitoring of patients with obstructive lung disease in a standardised way. This framework allows quantification of treatable traits (TTs) overlap in a real-life setting. Methods: Data collected between 2007 – 2023 were analysed. Patients ≥18 years of age attending the service for the first time, and receiving a working diagnosis of asthma, COPD, or Asthma-COPD overlap (ACO) from a pulmonologist, were studied. The occurrence and overlap of 7 TTs was studied: [1] Insufficient inhaler technique [2] Eosinophilic inflammation [3] Poor medication adherence [4] Obesity [5] Current Smoking [6] Reversible airflow limitation [7] Anxiety and/or Depression. Results: In total, complete data for the 7 TTs was available for 2,139 patients (asthma n=1,212; COPD n=669; ACO n=258). In total, 225 patients (10.5%) had no TT, n=591 patients (27.6%) had 1 distinct TT, and 1323 patients (61.9%) had ≥ 2 TTs. The highest co-occurrence of TTs were combinations consisting of the most prevalent TTs, see Figure 1. Conclusion: Most patients diagnosed with asthma and/or COPD typically manifest multiple treatable problems. A personalised approach targeting traits may be effective in achieving better control of these heterogeneous diseases. <fig><object-id>erj;64/suppl_68/OA1859/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
