A physiologist-led home spirometry assessment (PLHSA) facilitates respiratory diagnosis and treatment pathways

Introduction: Home spirometry can provide valuable insights to facilitate respiratory diagnosis and treatment optimisation, however data quality and adherence can be poor (Williams, ERJ Open Res 2023). Aim: To evaluate the adherence to and quality of a PLHSA and determine if it facilitates diagnosis and treatment approach. Methods: NHS asthma services referred patients to NuvoAir’s PLHSA. Patients were coached by NuvoAir physiologists to perform quality assured (QA) spirometry 4 times weekly and when symptomatic for up to 12 weeks. Post PLHSA, clinical reports were generated for referring clinicians to action. Results: Of 120 patients referred (91 Female) aged 46.5yrs (SD14.6), 8 withdrew. Reasons for referral: uncontrolled asthma with uncertain cause (68%), asthma uncertain with no evidence of obstruction (28%) and uncontrolled asthma with adherence concerns (4%). There was a median adherence of 75% (IQR 46-101) to 4 times weekly spirometry with 78% achieving QA spirometry. There were 200 outcomes of the PLHSA; thematically grouped (see figure). Diagnostic confirmation was achieved in 38%, medication optimised in 28% and onward referral facilitated in 26%. Conclusion: This PLHSA captured high quality data and adherence due to personalised support. In this cohort, within 12 weeks, the PLHSA provided evidence for onward referral, treatment optimisation and confirmation of diagnosis. Figure.
