Biomarker profiles across obstructive and non-obstructive lung function trajectories

Introduction: We recently presented lung function trajectories from a population-based cohort with annual spirometries in 993 obstructive and 993 non-obstructive individuals, spanning a decade. Three distinct trajectories were identified among obstructive subjects and two among non-obstructive. The extent to which these trajectories represent discrete endotypes is not clear. Aim: To explore a wide range of proposed biomarkers for lung function in samples collected at the onset of the identified trajectories. Methods: Collection of blood samples. Measurement of biomarkers; SLPI, hs-CRP, IL-6, IL-6R, IL-8, MMP-9, MMP-9/TIMP-1, SP-D, PAI-1, sRAGE, TIMP-1 and TNF-alpha. Non-parametric statistical tests. See also figure 1. Results: Biomarker data was available for 774 obstructive and 789 non-obstructive subjects. Among obstructive, significant differences between trajectories were found in IL-6, PAI-1, MMP-9, MMP-9/TIMP-1 and hs-CRP (p<0.001, p=0.018, p<0.001, p=0.001, p=0.002, respectively). In non-obstructive, IL-6, SP-D and SLPI displayed significant differences (p<0.001, p=0.007, p=0.016, respectively). Conclusions: Baseline biomarker profiles differ according to lung function trajectories in both obstructive and non-obstructive individuals, suggesting distinct pathophysiological processes that unfold over the decade following sample collection.
