Evaluation of annual mortality in patients hospitalized with COPD exacerbation
Introduction: We aimed to evaluate annual mortality data and related parameters in patients hospitalized with COPD exacerbation. Method: Patients hospitalized with COPD exacerbation between 01/06/2016 and 31/01/2021 were retrospectively evaluated as with or without all-cause mortality in the following a year after hospitalization. Results: The annual mortality rate in a total of 400 patients hospitalized with COPD exacerbations was 25%. When the data of mortal patients (n:101) and non-mortal patients (n:299) were compared; male gender, CCI(Charlson Comorbidity Index), LTOT(long-term oxygen therapy) at home, need for NIV(Noninvasive-Invasive Ventilation) on admission, and blood Urea and pCO2 values were found to be significantly high, while pH, Hb(hemoglobin), lymphocyte values were found to be significantly low. Additionally, the presence of pneumonia and pleural fluid on chest radiography was found to be significant in the mortal group. In ROC analysis, the parameters predicting annual mortality are; pH<7.38 OR:3.4, CCI >3.5 OR:2.9, Urea >50.25 OR:2.8, Lymphocyte <1.35 OR:2.2, hospitalization duration>11 days OR:2.9, Hb<13.3 OR:2.1, pCO2>57mmHg OR:2.4 (p values: <0.001, <0.001, <0.001, 0.002, 0.009, 0.011 and 0.010 respectively, Table 1) In multivariate regression analysis, male gender OR:3.55 95% CI(1.470-8.593 p:0.005), need for IMV on admission OR 95% 10.5 (3.107-35.526 p<0.001), pH<7.348 OR:2.2% 95 CI(1.099-4.562 p:0.026), Hb<13.35 OR: 1,978 95%CI(1,072-3,650 p:0.029) was detected. Conclusion: Patients hospitalized with COPD exacerbation; Those with pH<7.38mmHg, CCI >3.5, Urea >50 mg/dL, IMV requirement and Hb<13.35 may die within a year even if they are discharged
