Importance of subpleural fibrosis measured by a computer-aided system in patients with interstitial pneumonias, the prospective multicenter study.

Background: Quantitative measurements of subpleural fibrosis using a Computer-Aided system (GHNC) have been linked to the presence of a histologic UIP pattern and prognosis, (Iwasawa T. Radiology, 2019). Objective: This study aimed to assess the significance of subpleural fibrosis on CT scans in predicting overall survival. Methods: We analized 749 patients (550 males, median age 70 years) diagnosed with ILD, drawn from the nationwide prospective registry of idiopathic interstitial pneumonia (JIPS Registry). Utilizing GHNC system, we measured the ratio of fibrotic lesions in the 2-mm-width subpleural region (SF2) in initial CTs (Fig 1). We compared SF2 between patients with and without UIP pattern on CT using Mann-Whitney U-tests and assessed its correlation with overall survival through Cox regression analysis. Results: The median follow-up term was 4.9 years, during which 236 patients succumbed. SF2 was significantly larger in patients exhibiting a usual interstitial pneumonia CT pattern (42.9% vs. 32.1% with and without UIP pattern, p < 0.001). After adjusting for age and sex, SF2 emerged as a significant predictor, with a hazard ratio of 1.05 (95% confidence interval 1.03-1.06, p < 0.001). Conclusion: Subpleural fibrosis, as measured on CT scans, represents an objective measure for predicting the prognosis of patients with ILD. <fig><object-id>erj;64/suppl_68/OA5473/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
