Lung retrasplantation:the only hope for some patients
Introduction: Lung retrasplantation(LRT)is the only therapeutic option for lung transplant(LT)patients that presents a refractory failure of the graft.CLAD remains the leading cause of long-term morbidity and mortality for LT recipients. We have evaluated the survival outcomes ofLRT at a transplant center Methods: Retrospective analysis of patients who underwent LRT between 1993 and 2023. Specific data of patients were collected(table 1) <fig><object-id>erj;64/suppl_68/PA586/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> Survival outcomes were calculated. Results: A total of 16LRTwere performed of a total of 862LT(1.8%). The average age was 36.5years,8 were men.11 LRT were carried out due to CLAD.4 LRT airway complications(AC)and 1 LRT VDD. The mean period until LRT was 36 months in CLAD cases and 14 months in other cases. An increase in FEV1 of 30±12%was observed after LRT. The average survival was 32 months, slightly higher in CLAD patients(30 months),compared AC cases(26months)(p=0.1)(figure 1) <fig><object-id>erj;64/suppl_68/PA586/F2</object-id><object-id>F2</object-id><object-id>F2</object-id><graphic></graphic></fig> Conclusions: LRT represents a therapeutic alternative in LT patients with refractory failure of the graft. Those undergoingLRTinherently belong to a high-risk group, generally presenting less favorable outcomes. Careful selection of candidates is crucial, best outcomes are observed in CLAD
