Role of matrix metalloproteinase-1 (MMP-1) on lung remodelling in post pulmonary tuberculosis squelae

Background: Tuberculosis (TB) will leave residual symptoms even after finishing ATD treatment. Treatment of pulmonary tuberculosis can cause restrictive and obstruction that correlates with abnormal lesions in the lungs. Matrix metalloproteinase-1 plays a vital role in immunopathology and may be implicated in lung remodeling through the development of type I collagen breakdown and lung cavitation. Objective: To investigated the association between serum MMP-1 levels, lung function, and chest radiography in patients with post pulmonary TB sequelae. Methods: A total of 70 participants were enrolled: 35 post TB patients and 35 healthy controls. Spirometry was used to assess pulmonary function, ELISA was performed to measure MMP-1 levels, and chest xray was used to detect lung structure abnormalities. Result: Forced Vital Capacity (FVC) (p=0.000), Forced Expiratory Volume in 1 second (FEV1) (p=0.004), and FEV1/FVC (0.004) were significantly lower in post TB patients as compared to controls. Post TB radiographic findings included fibrosis (p=0.000), cavities (p=0.025), bronchiectasis (p=0.000), and atelectasis (p=0.011). No significant differences Serum MMP-1 levels between two groups (p=0.643). Bivariate analysis found significant corelation between MMP-1 to FEV1 (p=0,11; r=-0,426), FVC (0,005; r=-0,460) in lung function. MMP-1 levels and atelectasis were also correlated significantly (p=0.025; r=0,269). Conclusions: Althought there were no significant differences in MMP-1 levels compare to healthy subjects, MMP-1 levels showed a significant correlation with reduced lung function and chest radiological abnormalities.
