Sensitivity of high-sensitive cardiac troponin and NTproBNP as additional biomarkers in detection of cardiac sarcoid- experience in a UK centre
Introduction: Sarcoidosis is a multisystem disorder. New cases presenting to respiratory physicians should be screened for possible cardiac sarcoidosis (CS). In our service, screening has recently included assays of high-sensitive cardiac troponin T (hs-cTnT) and serum NTproBNP (N-terminal pro B-type natriuretic peptide). Method: We performed a retrospective analysis of patients discussed in the cardiac sarcoid multidisciplinary meeting from 2021 to 2022. Patients with evidence of active CS on fluorodeoxyglucose positron emission tomography (FDG PET) and/or cardiac Magnetic Resonance Imaging (CMR) with plasma hs-cTnT (n=15) and serum NTproBNP levels (n=14) were identified. Results: Hs-cTnT levels were raised in 3/15 patients (sensitivity 20%) NTproBNP levels were raised in 9/14 patients (sensitivity 64%). <fig><object-id>erj;64/suppl_68/PA721/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> Conclusion: In this cohort NTproBNP was more sensitive and hs-cTnT was less sensitive as a biomarker of active CS. Negative hs-cTnT does not reliably exclude CS and if assayed it should always be part of a panel of several biomarkers that should include symptoms, physical exam, ECG and echocardiogram leading to specialist imaging and specialist opinion when required.
