Remission of asthma in adolescents and young adults with severe asthma treated with Dupilumab

Introduction: Dupilumab is a fully human anti-IL-4 / IL-13 monoclonal antibody. It has shown efficacy in T2 severe asthma. We lack real-life studies addressing the achievement of clinical remission in patients treated with dupilumab. The period of adolescence and young adulthood (AYA) extends from 11 to 25 years of age as patients in this age group face similar challenges. Aim: Address remission of asthma in young people with severe asthma treated with Dupilumab. Methods: AYA patients with severe asthma treated with Dupilumab were evaluated. The main clinical, functional, and biological characteristics of severe asthma were assessed at baseline (T0) and after 1 year of treatment (T12). Clinical remission was defined at T12 in patients with no asthma exacerbations, no oral corticosteroid (OCS) use, ACT ≥ 20 and improvement in FEV1 ≥ 100ml from baseline. Results: 10 AYA patients, 60% female, mean age 20,2 years old were evaluated. 6 patients were naïve. Switch from omalizumab 3, switch from mepolizumab 1. Comorbidities were mainly T2 related: Food allergy 70%, atopic dermatitis 80%, allergic rhinitis 90%, eosinophilic esophagitis 30% and nasal polyposis 10%. The mean values of the T2 biomarkers are shown in the table. Among population, 8 patients achieved clinical remission at T12. Dupilumab got control in 9 patients, improved FEV1 >100ml in 10 patients and prevented exacerbations in 8 patients. One patient discontinued treatment due to local reaction at the injection site. Conclusions: Treatment with anti-IL4/IL13 can induce early clinical remission in most AYA patients with severe T2 asthma.
