Treatment and long term follow-up results in patients with pulmonary vascular thrombosis related with COVID-19

Backround: PE is a complication of COVID-19. Our study aimed to evaluate the prognosis and response to treatment in outpatients diagnosed with COVID-19related PE between 2020 and 2022. We investigated the incidences of chronicity, relapse, and mortality. Method: 101 patients who were diagnosed with PE and began anticoagulation during the COVID-19infection period or within the first month after the COVID-19 among those applying to the our outpatient Clinic, with COVID-19 PCR positivity, were recorded. The patients' comorbidities,PESI score,PE diagnostic method, biochemistry, and transthoracic echocardiograph parameters at the time of diagnosis and the 24th month of follow-up were recorded. Cardiac catheterization parameters were recorded. All parameters at the time of diagnosis and the 24th month of follow-up were compared between the study group and the 77 control groups. Results: Among groups, gender, age,BMI,and chronic disease score were found to be similar. The diagnosis rate with Q-SPECT was higher in COVID-19related PTE patients(p<0.001). Patients rate with DVT was identical between the groups. In the study group, 43.6% of the patients received anticoagulant treatment for three months. After the 3rd month, patients in the study group received 49.1% LMWH,50.9% DOAC anticoagulation. At 24 months, the rate of patients continuing treatment was similar between the study and control groups. The sPAB value in the CTED group was higher than the group responding to PTE treatment (p:0.009). No side effects of anticoagulant treatments were observed during the treatment. Conclusion: In the 24-month follow-up patients, death was detected in 2%,relapse in 2.2%,CTPEH in 4.9% of and CTED in 9.9%.
