Multidrug-resistant tuberculosis and diabetes mellitus

Tuberculosis (TB) and diabetes mellitus (DM) are among the significant modern medicine’s problems that have a direct impact on the societies’ socio-economic indicators. The problem of diabetes as well as tuberculosis is more acute in countries where tuberculosis is epidemic and the burden of diabetes tends to increase linearly. Materials and methods: Our study is based on an analysis of statistical data obtained from a retrospective study of 762 case histories and cases of MDR-TB in the register of tuberculosis patients in different regions of Ukraine for 2017-2021. The main epidemiological aspects of the prevalence of diabetes in patients with MDR-TB, the occurrence of adverse reactions to TB therapy (PPT) in such patients as well to evaluate the effectiveness of treatment with standard schemes of such polymorbidity. The rate of successful treatment in gr.2 is probably higher than in gr. 1 (64.7% vs. 61.4%; (p <0.05)). However, a more significant probable difference is characterized by the treatment rate, which in the main group is 27.3% versus 40.3% in the control group (almost 2 times; p <0.05). The rate of ineffective treatment, which in patients with comorbidity was 27.3% (almost every third patient) against 17.6% in group 2 is also important for scientists and practitioners. Conclusions: The association between diabetes and tuberculosis can be the next challenge in global TB control worldwide, and untimely diagnosis of poverty can have a major impact on quality of life and economic productivity. The combination of MDR-TB / DM significantly worsens the course and prognosis of the latter, as proved by our prospective study.
