Late Breaking Abstract - Implantable Artificial Bronchus (IAB) Performance up to 1 year
The IAB is a self-expanding airway stent placed bronchoscopically in severe emphysema patients to prevent expiratory collapse, reducing RV by releasing hyperinflated air. The IAB is made from polyether ether ketone (PEEK), a biocompatible polymer. In the IAB-1 trial (<ext-link>NCT05087641</ext-link>), the stent was examined with a thin bronchoscope at 30- and 90-days. RV is considered a surrogate for airway patency. If the change is stable, the airway remains unobstructed. Methods: Airway patency is examined at 30 days after the initial implant(s) at which time additional implants are allowed. A final bronchoscopic evaluation occurs at 90-days, accompanied by efficacy endpoint evaluation. Follow-ups at 180-days and at 1 year. Results: No obstruction or granuloma were observed in 77% (20/26) IABs at 30-days and in 79% (30/38) IABs at 90-days. Endpoint RV reduction exceeded the MCID (≤0.310L) for 73% (11/15). At 180-days, the response rate is 71% (5/7) and at 1 year, 67% (2/3). Images (Fig. 1) demonstrate an open airway, collateral access to the airway, unobstructed distal end, and biologic accommodation—endothelialization— to the PEEK filaments: at 30-days, the stent filaments lie against the airway surface creating a “cobblestone” appearance; at 90-days, the surface is smoother; at 365 days, endothelization has advanced with clear collaterals. Conclusion: IABs reduce RV volume in a majority of patients and it seems stable over the first year.
