Impact and burden of T2-low severe asthma: Insights from the Greek PHOLLOW Study
Background: Data on type 2 (T2)-low severe asthma (SA) is scarce due to the absence of a consistent classification system, and therapeutic needs on this population remain unmet. Objective: To assess the frequency, management strategy and burden of T2-low SA in Greece. Methods: A multicenter study conducted from March ‘22-March ‘23 in routine care settings across Greece. Based on a newly proposed classification system, patients were classified as T2-low if blood eosinophil count (BEC; cells/μL)/fractional exhaled nitric oxide (FeNO; ppb)/allergy status were <150/<25/any or <150/<50/negative or <300/<25/negative. Additional outcomes included: Asthma Control Test (ACT), Mini-Asthma Quality of Life Questionnaire, Hospital Anxiety and Depression Scale, and Work Productivity and Activity Impairment:Respiratory Symptoms questionnaire. Results: Of the 602 enrolled SA patients, 20.1% were classified as having T2-low SA. Of those, 71.1% had experienced ≥1 clinically significant exacerbations in the past 12 months, 62.8% had ACT score <20, and 22.3% were receiving biologics. Among evaluable patients, 79.5% reported impaired quality of life due to asthma, and 43.8% suffered from clinically significant emotional distress, while 30.0% of patients presented activity impairment and work productivity loss. Clinical and patient-reported outcomes were worse among patients with ACT-defined uncontrolled asthma. Conclusions: One-fifth of SA patients present with a T2-low endotype, based on our classification system. This, along with the uncontrolled disease and life impairments experienced by the patients, underscore the validity of current diagnostic tools and the need for novel therapeutic approaches in this population.
