APP.nee: a digital tool for obstructive sleep apnoea screening

Background: The rising incidence of obstructive sleep apnoea highlights the crucial role of early screening in identifying the disorder, enabling timely treatment to enhance quality of life and lower the risk of associated comorbidities. Methods: In 2021, the Italian Apnoeic Patient (Apnoici Italiani) introduced 'APP.nee,' a WebApp featuring validated self-administered questionnaires (SANReSP, Epworth Sleepiness Scale, and Insomnia Severity Index) for the initial screening of adult sleep respiratory disorders. Following questionnaire assessment, patients were provided with a list of designated sleep centers nationwide for further diagnostic confirmation. Results: From 2021 to 2023, the app was used by 805 adults (M: 71.8%; F: 28.2% age 48.7 ± 13.1 yr). The 24.5% of subjects showed an ESS>10 and 63.8% an ISI>14. An 8 items questionnaire was sent to 200 subjects to investigate the diagnostic and therapeutic path undertaken. Of these 20 subjects (16 M, 4 F) answered in the following 5 days (February 3/7-2024). 15 patients had a moderate-severe risk of OSA, 4 mild risk and 1 no risk via SANReSP questionnaire. After nocturnal polygraphy, 10 patients showed AHI >30; 5 AHI>15; 2 AHI>5; 3 AHI <5. Of the 15 subjects with moderate-severe risk, 85% showed an AHI>15 with polygraphy examination. After diagnosis, 14 patients chose positive pressure devices for therapy, 1 underwent surgery, and 5 did not start any therapy. Comments: The adoption of App.nee for OSA screening represents a modern, accessible, and effective method for early disorder detection. The integration of easy to use, immediate results, and detailed information not only streamlines the diagnostic process but also enhances patients awareness of their sleep disorder
