The impact of clinical features on prognosis in advanced-stage lung adenocarcinoma
This study aims to explore the predictive value of patients' gender, smoking history, comorbidities, performance status at diagnosis, and laboratory parameters (lactate dehydrogenase, C-reactive protein, hemoglobin, neutrophil/lymphocyte ratio) for progression-free and overall survival in advanced-stage lung adenocarcinoma. Average PFS was 23.35 months, and OS was 37.17 months. Actively smoking patients had significantly shorter OS than ex-smokers (p<0.05). Patients with CCI >3 and ECOG PS >2 had lower PFS and OS. For CCI 0-3, PFS was 26.06 months, OS 42.36 months; for CCI >3, PFS was 13.23 months, OS 18.30 months (p<0.05). ECOG PS 0-1 group showed PFS 26.63 months, OS 42.36 months; PS >1 group had significantly lower values (p<0.001). Deceased patients were mostly in stage IIIC, IVA, and IVB (p<0.001). PFS for stages IIIC, IVA, and IVB were 12.87, 16.00, 9.57 months; OS were 20.00, 24.63, 16.67 months, respectively (p<0.001). <fig><object-id>erj;64/suppl_68/PA2423/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> In this study examining advanced-stage lung adenocarcinoma patients, it was demonstrated that those actively smoked, had an ECOG PS >1 and CCI >3 at the time of diagnosis, exhibited significantly lower progression-free and overall survival. It was observed that CRP, NLR, Hb, LDH were found to be higher in deceased patients when heading for diagnosis.
