Outcome after treatment of malignant pleural effusion (MPE) with indwelling pleural catheter (IPC). Multicentric study (PLEURORAPID trial: PI21/01420)

Treatment of MPE with IPC offers a first-line alternative. Research needs to develop strategies that induce pleurodesis and catheter removal. Objective: To describe the results of managing MPE with CPT and determine wich factors can influence the control of MPE. Methods: Ambispective study in 4 Spanish Hospitals (April/2022-August/2023). It was evaluated which factors (clinical, caractheristics of MPE, follow-up/management strategy of the IPC) could be related to the procedure outcome (pleurodesis/complications), IPC removal or the need for new procedures. Results: 77 IPC, 57% women, 72 (65-79.5) years old, 41% adenocarcinoma, 23% trapped lung (TL). CPT was removed in 44% due to pleurodesis (37 days) and 13% to complications. 9% needed new procedures. In patients with intensive drainage±talc (58%vs22%;p=0.002) and if the indication for IPC was by first-line (74%vs27%;p=0.001) pleurodesis was more frequent. Hypoalbuminemia was associated with no pleurodesis (35%vs7%;p=0.006) and oncological treatment with pleurodesis (86%vs65%;p=0,04). IPC as first-line treatment showed to be independent predictor of pleurodesis (OR=7;p=0,001). 15% patients presented mild complications and 10% were hospitalized. Neither comorbidity, days of IPC or management strategy were associated with risk of complications, only TL was independent risk factor (OR=4.7;p=0.01). Conclusions: IPC is a effective treatment for MPE, pleurodesis is the most frequent cause of removal. New pleural procedures are infrecuent. IPC as first-line treatment is the most significant predictor pleurodesis. The percentage of complications is low but they can require hospitalization
