Reducing pulmonary hemorrhage risk in CT-guided lung biopsy - impact of ground-glass opacification in the access pathway and lung gravitational physiology.

This study aimed to reduce pulmonary hemorrhage risk during CT-guided lung biopsy by assessing target lesion characteristics and lung gravitational effects through patient positioning. Materials and Methods: We analyzed 136 CT-guided lung biopsies from January 2020 to December 2023, categorizing patients by hemorrhage severity. Potential confounders including demographics, lesion details, biopsy specifics, emphysema presence, and ground-glass in the access route were evaluated. Student t-test and Fisher’s exact test compared patient characteristics and hemorrhage occurrence, while multivariable logistic regression identified independent factors. Results: Higher-grade hemorrhage was significantly associated with lesions in dependent lung area (OR 3.510, 95% CI 1.493-8.252, P<0.001) and ground-glass presence in the access route (OR 2.975, 95% CI 1.261-7.02, P<0.015). No increased higher-grade hemorrhage was noted in benign or malignant lesions. Conclusion: Awareness of ground-glass in or around target lesions in the access route and proper patient positioning can preventively reduce the incidence of higher-grade pulmonary hemorrhage during CT-guided lung biopsy. <fig><object-id>erj;64/suppl_68/PA2564/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
