Cardiovascular disease risk score assessment in obstructive sleep apnea (OSA) a case-control study.

Introduction: Individuals with severe sleep apnea are at increased risk for cardiovascular diseases. We aimed to find out correlation of commonly used cardiovascular score among OSA patients and matched control. Methodology: Adult (age >18 years) will undergo screening for OSA using STOP BANG and BERLIN questionnaire. Patients with low risk for OSA were taken as controls patients confirmed to have OSA by Polysomnography were taken as cases. All Cases and controls are then subjected to undergo cardiovascular (CVS) risk scoring using Framingham risk scoring questionnaire 2008. Multivariable regression analysis was done to find out correlation of individual variables and Cardiovascular risk score. Results: There was 120 cases and 120 control with mean age +SD of cases and controls were 54.6+-11.8 , 51.2+-10.6 respectively, Baseline variables were equally matched between cases and controls. Multivariable regression analysis showed significant correlation of systemic hypertension(p=0.001),BMI(p=0.001), CRP (p=0.014), HbA1c (p=0.005) in cases and stop bang (p=0.001) in control with Cardiovascular risk score. Association between Cardiovascular risk and AHI,oxygen desaturation index were not statistically significant. Summary: Commonly used framingham risk score will underestimate cardiovascular risk score in OSA patients. Prospective cohort study is ideal method to confirm association of sleep study variables and cardiovascular risk. Reference -Shahar E, Whitney CW, Redline S, Lee ET, Newman AB, Nieto FJ, O'Connor GT, Boland LL, Schwartz JE, Samet JM.
