Stoma Stent in a traqueotomy decanulation process. Experience in a cohort of patients with prolongued mechanical ventilation.
Despite the favorable implications of the use of a stoma stent in the decannulation process, it is not present in any protocol published to date. Objective: To analyze the characteristics of the patients and the results obtained with the use of the Montgomery cannula(MC) in the first cases performed in our center. Retrospective study of patients who had a MC placed during the decannulation process from Dec/18 to Oct/23 in the Pneumology Department. The indication of MC was established after assessment by the Pneumology team specialized in the management of the patient with noninvasive mechanical ventilation (NIV) needs. Its placement was performed when the patient met decannulation criteria (48h with the cannula plugged);under fibrobronchoscopy vision and superficial sedation. 39 patients (71.8% male), the baseline characteristics of the patients are described in Table 1. In 30.8% of cases the reason for orotracheal intubation was multifactorial. The reason for tracheostomy in 56.4% of cases was myopathy. Decanulation was successful in 94% (33/35) of the patients and reintubation was avoided in 14% (5/35) of the cases, as shown in Figure 1. Only the previous days with non-invasive mechanical ventilation predicted the possibility of hospital discharge without a tracheal button. The use of the Montgomery cannula as an intermediate step in the decannulation process for patients with prolonged weaning is a safe technique that allows definitive decannulation in most cases.
