Erythropoiesis disorders in COPD
Introduction: Chronic obstructive pulmonary disease (COPD) is now considered a systemic disease. Abnormalities observed in the erythroid lineage are common. Many factors associated with COPD interact with erythropoiesis: smoking, hypoxemia, systemic inflammation, infectious exacerbations. Methods: Retrospective study from January 2018 to December 2023, covering 350 patients with COPD collected in the respiratory diseases department of the Ibn Rochd University Hospital in Casablanca. Results: Anemia was noted in 18% of patients with COPD. It was microcytic hypochromic in 81% of cases. The average hemoglobin level for these patients was 11 g/dL. Furthermore, polycythemia was noted in 11% of cases. The other associated co-morbidities were dominated by arterial hypertension in 34% of cases, obesity in 9% of cases, diabetes in 6% of cases and dyslipidemia in 21% of cases. All our patients were placed on background treatment. Iron supplementation was recommended in 15% of cases. The outcome was good in 65% of cases and marked by frequent exacerbations in 18% of cases. Conclusion: Erythropoiesis disorders associated with COPD are more common in our context. This can be explained by the diagnosis of COPD often at a late stage.
