The role of the "nurse navigator" in implementing and operatining a lung cancer screening consultation for high-risk smokers

Aim and objectives: Describe the role of the “nurse navigator” in a screening consultation for high-risk smokers with chronic obstructive pulmonary disease (COPD) and/or emphysema, and present the initial results. Methods: In April 2023, a lung cancer screening (LCS) consultation began at Santa Maria University Hospital in Lleida. This initiative evaluates smoking patients meeting lung cancer screening (LCS) criteria, including those with COPD and/or emphysema. The evaluation encompasses lung function testing, alpha-1-antitrypsin deficiency (DAAT) detection, and a comprehensive health questionnaire following the international early detection program for LCS (I-ELCAP). The “nurse navigator” play a crucial role, managing data collection, conducting respiratory function tests, and overseeing and supporting patients throughout the LCS process. Results: A total of 70 patients were assessed, predominantly men (78%) and former smokers (61%). Among them, 53% had COPD, while 43% had emphysema without airflow obstruction. Mutations associated with DAAT were detected in 15 patients (12 M/S, 2 S/S and 1 M/I variants). Suspicious lung lesions were detected in two patients, who subsequently underwent surgical treatment. The final pathological diagnoses were adenocarcinoma and squamous cell carcinoma, both in stage IA. Following surgery, these patients did not require additional treatment and are currently undergoing joint monitoring through LCS consultations and thoracic surgery. Conclusions: The “nurse navigator” enables thorough patient assessment, providing personalized education and management to address LCS complexities and anticipate individual needs.
