EVALUATION OF A CUT-OFF POINT IN THE FEOS SCORE FOR ASSESSING RESPONSE TO BIOLOGICS IN PATIENTS WITH SEVERE ASTHMA

Introduction: Clinical practice guidelines in non-controlled severe asthma recommend the evaluation of clinical response to biological therapy. Upham et al. criteria and FEOS score are tools that permit us to measure the biological response, however, we do not have cut-off points that confirm adequate response in FEOS score. Patients and Methods: This is a one single-center, observational retrospective study of severe asthma patients treated with anti-IL-5/IL-5R biologics. We applied Upham et al. criteria and FEOS score after one, two and three years of treatment. We validated FEOS score in our population and searched for a cut-off point for adequate response in FEOS score. Results: We included 60 patients (32 treated with mepolizumab, 18 with benralizumab and 10 with reslizumab). One year post-treatment, the FEOS median was 76 and 30 patients were super-responders. The association between FEOS and super-responders obtained an area under the ROC curve of 0,786 (image 1). FEOS=75 can be the cut-off point to identify super-responders, its sensitivity was 80 % and the specificity was 59,3 %. Two years post-treatment, FEOS median was 76 and there were 24 super-responders. The association between FEOS and super-responders obtained an area under the ROC curve of 0,835 (image 1). FEOS=75 could be the cut-off point for super-responders, the sensitivity was 80 % and the specificity of 59,3 %. Conclusions: This is the first time that a cutt-off point in FEOS for adequate response is obtained, FEOS=75 could be the suitable cut-off point, and we believe it will be very useful for clinical practice to maintain or remove the biological treatment.
