Pulmonary vascular resistance to predict one-year survival in ILD-PH

Pulmonary hypertension (PH) is a deadly complication of interstitial lung diseases (ILD). With the advent of new therapies, the need for short-term survival prognostic factors has become critical. We assessed whether the level of pulmonary vascular resistances (PVR) is a discriminating factor in one-year survival. We analyzed data of patients with ILD-PH from the French national prospective PH registry. Results are given as median [IQR]. The ability of PVR to predict one-year mortality was evaluated using the time-dependent area under the receiver operating characteristic (ROC) curve (AUC) and the associated 95% confidence interval (95%CI). N = 581 patients were included (male 78%, aged 70 years [64 – 77]). PVR were 6.8 wood units (WU) [5 – 9.5 WU]. N = 148 (25%) had PVR ≤ 5 WU, n = 208 (36%) had PVR > 5 and ≤ 8 WU and n = 214 (37%) had PVR > 8 WU. PVR were a long-term prognostic factor with a median time to death of 26.2 months [20.5 – 32.3] if PVR ≤ 5UW, 17.8 months [14.7 – 24.4] if PVR > 5UW and ≤ 8UW and 16.4 months [13.8 – 19.9] for patients with PVR > 8UW (p < 0.001). However, the PVR value is not able to discriminate patients that will die during the year from those that will survive (AUC of 0.56 [0.51 – 0.61]). PVR insufficiently predicts one-year survival in ILD-PH. Additional studies are required to enhance our ability to predict short-term outcomes in this life-threatening condition. <fig><object-id>erj;64/suppl_68/PA3417/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
