Late Breaking Abstract - Reconstructing a Lung Cancer Screening Program to improve early stage lung cancer detection

Introduction: The State of Nevada has one of lowest lung cancer screening rates in the United States, ranking 47th. 1.4% high-risk patients are screenin in Nevada, compared to national average of 4.5%. Aim: To surpass the 1.4% lung cancer screening rates in one year and increase early stage diagnosis of lung cancer in Norther Nevada. Methods:Initiated a provider run lung nodule screeening program on 4/2023. Steps implemented to improve and support screening included: Return on investment analysis (ROI) to support salary of provider, collaborating with quality team to ensure pack year accurately documented in electronic medical record (EHR), IT algorithm to alert primary provider that patient qualifies for screening, and automated work flow algorithm implemeted in EHR for notification when lung cancer screening request placed. The provider followed all enrolled patients and directed RAD 4 to pulmonologist to be seen in 2 weeks. Results: Complete data sets available from 11/2023 to 04/2024. The total lung cancer patients eligible for screening was 3,995 of which 20.4% completed shared decisionn making and qualifying screening CT. Of the 806 screened patients, 2.1% were diagnosed with lung cancer Stage I or II. The ROI extrapolated for 12 months exceeded the targeted goal by 55%. Conclusion: Changing to a provider led program and using automated algorithms within the EHR, we surpassed Nevada's and national screening rate by 14% and 4.5% respectively. Compared to National Lung Cancer Screening Trial of 2011, for every 320 sceening CTs we more than doubled the lives saved. To further increase our lung cancer screening we have now added IRB- approved word processing Artificial Intelligence program.
