Effectiveness of switching to Dupilumab from other biologics therapy in patients with severe asthma.

Objective: to evaluate the effectiveness of switching to dupilumab from a previous biologic in patients with severe uncontrolled asthma in a real-world setting. Material and methods: an observational, descriptive and ambispective study was carried out on patients that were switched to dupilumab from other biologics. Demographic and clinical data, pulmonary function test, asthma control test (ACT), number of exacerbations and hospital admissions were recorded at baseline and after 6 months of follow-up. Results: 21 patients were enrolled (6 men and 15 women), mean age of 52.7±13.8 years. Regarding comorbidities, 11 patients had allergic rhinitis, 9 nasal polyposis, 6 bronchiectasis, 7 gastroesophageal reflux, 3 osteoporosis, and 2 anxiety and/or depression. Ten were corticosteroid dependent. Three patients had been previously treated with mepolizumab, 11 with omalizumab, 5 with benralizumab and 2 with reslizumab. The most frequent reason for switching was the lack of response. A significant reduction was observed in the rate of exacerbations of 75.13% (3.86±2.5 vs 0.96±1.03 p<0.0001), in emergency visits (1.90±2.04 vs 0.76±1.2 p<0.001), hospitalizations (0.48±1.03 vs 0.18±0.39, p<0.047) and number of cycles with oral corticosteroids (3.78±3.2 vs 0.57±1.01, p<0.0001). Improvement was observed in mean FEV1 (1845.05±795.7 ml vs 2235.38±777.46 ml, p<0.0001), mean ACT score (10.00±4.5 vs 17.75±7.4 p<0.0001) and IgE levels (525.7±729.8 vs 100.2±167.1 p=0.014). Conclusions: Switch to dupilumab provided a significant lower exacerbations rate, hospitalizations and need for oral corticosteroids, as well as better lung function and asthma control.
