Value-Based Healthcare implementation by homecare providers for chronic respiratory patients

Introduction: Respiratory chronic conditions represent a high burden for healthcare systems. To address these, the Value-Based Healthcare (VBHC) approach ensures better correlation between patient outcomes and costs, which homecare providers can actively support by: personalizing patient support collecting patient outcomes, and adapting support over time based on patient needs Different studies have positively demonstrated the value of this support. However, scaling up live implementation remains a challenge as it requires significant human and technical resources. Method: This evaluation explores homecare provider implementation of Positive Airway Pressure (PAP) and non-invasive ventilation (NIV) therapies with personalization through outcomes collection. This was executed in two levels: Level 1 ‘Device-Centric’: reactive personalized support based on compliance and/or AHI, and/or leaks. Level 2 ‘Outcome-Centric’: predictive personalized support based on clinical outcomes (CROM) and patient reported outcomes (PROM) (table 1). Literature review and patient surveys on outcomes that matter for them were studied and chosen with clinicians. According to patient profiles, personalized care plans are defined: frequency of interactions, educational support, and coordination with healthcare professionals. Results: The number of patients under level 1 in PAP and NIV in 20 countries is already significant, respectively 66% and 40%, whereas the implementation of patients under level 2 remains limited with respectively 2% and 0,4% (table 2). Conclusion: The implementation of VBHC with a focus on personalization and systematic outcome monitoring is ongoing. Next steps will include a real world study.
