Type-2 comorbidities and severe asthma in a tropical middle-income country: the ISAR Colombia experience

Introduction: In patients with severe asthma (SA), type-2 comorbidities (T2C) could negatively impact the disease, making it necessary to describe its prevalence and its effect, particularly in tropical middle-income regions, where the available data are scarce. Methods: Cross-sectional study in adults with SA included in the International Severe Asthma Registry (ISAR) in Colombia, 2019 to 2023. The baseline demographic and clinical characteristics of patients with and without at least one T2C (allergic rhinitis [AR], chronic rhinosinusitis with or without nasal polyps [CRSwNP / CRSsNP] and atopic dermatitis [AD]) were compared. Results: We included 516 patients, 381 (74.3%) women, 379 (73.5%) had at least one T2C, with the following prevalences: AR: 61%; CRSwNP: 21%; CRSsNP: 21%; AD: 13% (Figure). Among non-T2C: obesity: 32% and obstructive sleep apnea: 26%. Patients with T2C were younger (p=0.025), more frequently women (p=0.002), had an earlier asthma onset (p=0.031) and higher blood eosinophil count ≥300 cells/µL (<0.001), and were more likely to receive biologic therapy (BT) (p=0.016). Conclusion: In a tropical middle-income country, the prevalences and characteristics of T2C in patients with SA are similar to those observed in developed nations. Patients with T2C are more frequently treated with BT. <fig><object-id>erj;64/suppl_68/PA3992/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
