Impact of disposables head-to-head to e-cigarettes, and combustible cigarettes on arterial stiffness

Introduction: E-cigarettes continue to be controversial in science and especially for smoking cessation. A new trend has spread among young people and adolescents - disposables, in which the nicotine-containing liquid is in a cartridge as a disposable product and advertises with colorful and fruity aromas. We investigated the acute effects of two different ELFBAR disposables head-to-head to combustible cigarette and a e-Cigarettes (MyBlu) with nicotine (18 mg/ml) on vascular function. Methods: This single-center four-arm cross-overstudy included 30 healthy occasional smokers (24.4±2.2 years) with at least 3 nicotine contacts per week. The participants had to consume each device. We measured peripheral and central hemodynamics as well as vascular function (Mobil-O-Graph™) before, seven, 15 and 30 minutes afterwards. Results: All 4 devices significantly increased SBP, DBP and PP as well as Heart rate during the follow up of the 30 minutes. Likewise, the parameters of central hemodynamics increased significantly, including central SBP and central DBP. For arterial stiffness, represented by AIX@75 and PWV, significant increases and thus a significant deterioration could also be shown. Peripheral vascular resistance also increased significantly in all groups. The 4 groups did not differ significantly in the parameters analyzed. Conclusion: Disposables induce vascular dysfunction like normal e-Cigarettes and combustible cigarettes, which indicates an increased cardiovascular risk and no harm reduction compared to combustible cigarettes. In addition, no differences in vascular effects could be shown for the two different flavors of the ELFBAR.
