Implementation of Shared Decision Making in monitoring patients with lung disease and/or heart disease- ADLIFE project
Introduction: In the EU ADLIFE project Shared Decision Making (SDM) is a core concept with the purpose of giving patients with chronic diseases eg. COPD and/or heart failure, the opportunity and power to participate constructively and actively in the decision-making processes regarding the mangement of their health condition. A SDM takes into account both the professional and scientific angle, as well as the patient's values and preferences. In order for healthcare professionals to use SDM, training in the method is required. Objective: To implement SDM in the digital ADLIFE platform Method: In order to ensure the use of SDM in the ADLIFE intervention, various initiatives were initiated: Education of health professionals, preparation of material for patient and health professionals. Results: Educational material has been developed, training of ADLIFE partners via live webinar, e-learning program with video for healthcare professionals, decision support tools for choosing inhalation devices have been developed. Additionally, trigger points which identify opportunities for the use of SDM in the patient process were developed (Fig. 1). <fig><object-id>erj;64/suppl_68/PA4151/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> Conclusion: SDM is built into the ADLIFE platform and will be tested in spring/summer 2024. SDM incorporated into a digital platform is a new method to ensure the use of SDM in patient processes and will influence the approach to patients in clinical practice in the future.
