Diaphragmatic surface EMG and COPD Inflammatory profile

INTRODUCTION Skeletal muscle dysfunction is one of the systemic complications of COPD that influences morbidity and mortality. This study aims to determine the relationship between diaphragmatic dysfunction and systemic inflammation in COPD, assessed by diaphragmatic surface EMG and biomarkers of systemic inflammation. METHOD Forty male patients with COPD (GOLD criteria) in stable clinical condition were included prospectively. COPD patients had benefited, among other things, from a Diaphragmatic surface EMG, an assessment of the inflammatory state (CRP, IL-6, TNF-α), and muscle biomarkers (CPK, LDH). RESULTS: • The forty male COPD patients were mainly aged 60 to 70 years, weighing an average of 69.5 kg and measuring an average of 1.70 m. • The frequency of pathological diaphragmatic surface EMG was 63.2%. • The frequency of the decrease in amplitudes associated with increasing latencies was 36%. •The frequency of muscle biomarker abnormalities was 10.25%. • Abnormalities of surface EMG of the diaphragm associated with muscle biomarker abnormalities in 24.5% of cases. • Positive correlation between inflammation biomarkers and myogenic damage of the diaphragm. • Circulating cytokines significantly correlated with myogenic damage of the diaphragm, particularly IL-6. • Circulating IL-6 was higher in COPD patients with collapsed amplitude. CONCLUSION At the end of these observations, it must be emphasized that diaphragm myogenic damage is frequent.
