Multifaceted analysis of serum biomarkers for autoimmune features in patients with idiopathic interstitial pneumonia: A post-hoc analysis of prospective multicenter cohort study

Background: Interstitial pneumonia with autoimmune features (IPAF) is attracting an attention in patients with idiopathic interstitial pneumonia (IIP). However, details of biomarkers and the treatment response in these patients have not been fully elucidated. Objective: To find useful serum biomarkers that reflect connective tissue disease (CTD)-related features and the classification of IPAF in patients with IIP. Methods: Newly diagnosed patients with IIP were prospectively enrolled in this multicenter cohort study. In post-hoc analysis, serum levels of C-X-C motif chemokine (CXCL) 10, CXCL1, CCL2, BAFF, leptin, and angiopoietin-2 were measured by enzyme-linked immunosorbent assay. Results: Two hundred twenty-two patients with IIP were enrolled between 2015 to 2020. The median observation period was 36 months. The proportion of IPAF was 11.7% (26 patients). In patients with high serum CXCL10, changes in both %FVC and %DLCO in one year significantly increased (p=0.014 and p=0.009, respectively), although other biomarkers did not. A higher proportion of patients with high CXCL10 were diagnosed as NSIP with OP overlap (p=0.003), had CTD-specific autoantibodies, and met IPAF classification criteria (p=0.003) than those with low CXCL10. When limited to patients with IPAF, patients with high CXCL10 showed higher increase in %FVC in one year (p=0.029). Finally, in multivariate analysis, high serum CXCL10 was a significant favorable prognostic factor (HR 0.368, p=0.005). Conclusions: High serum CXCL10 was associated with CTD-related features and the classification of IPAF in patients with IIP.
