Late Breaking Abstract - Eligibility for lung cancer screening among incidental lung nodule patients in a major healthcare system

Background: The uptake of lung cancer screening (LCS) remains low, whereas incidental lung nodule (ILN) detection is rising. Managing ILNs presents an opportunity to identify LCS candidates. Methods: Utilizing natural language processing, we analyzed CT scan reports from January and February 2022 at Mount Sinai Health System for ILNs. Data on smoking history and previous LCS participation were extracted. Patients lacking smoking history were excluded. Results: Of 1,947 patients identified with ILNs, 479 (24%) were LCS-eligible with a median age of 67 years. Smoking status was split between 200 (42%) current and 279 (58%) former smokers. Among those 479 patients, 328(68%) had undergone at least one LDCT, with 282 (86%) continuing in the LCS program, 35 (10.6%) dropping out, and 11 (3.4%) scheduled for their next LDCT. Notably, 59 (21%) of the continuing participants had no LDCT in the past two years, and 151 (31.5%) of the eligible had never undergone LDCT. <fig><object-id>erj;64/suppl_68/PA4218/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> Conclusion: ILN management programs effectively identify potential LCS candidates, including those previously non-participative or overdue for LDCT. A substantial proportion of ILN patients qualify for LCS.
