EARLY ARREST OF LUNG DEVELOPMENT AND ITS IMPACT ON CHILDREN’S AIRWAY RESISTANCE

Background: Bronchopulmonary dysplasia (BPD) is one of the most common and serious sequelae of premature birth. It is well documented that BPD patients have significantly reduced lung function compared to children without BPD diagnosis. Aim: Describe a cohort of pediatric patients with a history of BPD, including impulse oscillometry (IOS) findings and compare them with healthy children in a high-altitude city. Methods: Retrospective analytical cross-sectional study including pediatric patients (3-5 years) with a history of BPD evaluated between 2019 and 2023 in Bogotá, Colombia, compared to healthy children of the same age and sex who were previously included in a IOS reference values study in our institution. Results: Seventy-one patients (64%) had BPD diagnosis. Most were female (50.7% in BPD vs 57.5% in the healthy group). The median gestational age in the BPD group was 31 weeks (IQR 28-32 weeks) with a median birthweight of 1350 grams (IQR 993-1800 grams). Most BPD patients received inhaled corticosteroids (56.3%) and 12.7% received montelukast. Just as well, most BDP patients had no hospitalizations due to respiratory causes in the last year (69%) and 21.1% had one. With regards to IOS values, mean pre-bronchodilator and post-bronchodilator values for R20, R5-R20, X5, and AX were significantly higher in the BPD group. There was no significant difference after bronchodilator. Conclusions: In this study it was found that children under 5 years of age with BPD history had fixed peripheral obstruction compared to healthy children. No significant difference was found in response to bronchodilator.
