Proportion of non-ventilator associated Hospital Acquired Pneumonia associated with sepsis
Introduction: Hospital Acquired Pneumonia (HAP) is the most common Healthcare Associated Infection. (Suetens, C et al. ECDC. 2023) and most are non-ventilator associated (nv-HAP). If complicated by sepsis, optimal management requires rapid administration of appropriate antimicrobials and supportive care. In contrast to community acquired pneumonia, the incidence of sepsis in nv-HAP is poorly described; estimates from administrative data have poor generalisability due to the limitations of coding. (Carey E et al. Am J Infect Control. 2022; 50(12):1339-45). Aim: To describe the proportion of nv-HAP cases with sepsis. Methods: Patients suspected of HAP were enrolled into a 4 centre, randomised trial in the UK (https://clinicaltrials.gov/study/<ext-link>NCT05483309</ext-link>), from which, clinically and radiologically confirmed cases were analysed. Sepsis was considered present, if acute organ dysfunction was evident according to eSOFA criteria (Rhee C et al. Crit Care Med. 2019;47(3):307-314). Results: The findings for 120 patients are shown in the table. Sepsis was not associated with in-patient mortality odds ratio 2.9 (p=0.07, CI 0.9, 9.5 <fig><object-id>erj;64/suppl_68/PA5072/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> *>40% FiO2 for > one hour # Doubling of creatinine / halving of eGFR $ Assessed by: A (alert) V (responds to voice) P (responds to pain) U (unresponsive) Conclusions: Sepsis complicates nv-HAP in a quarter of patients. Recognising HAP as a cause of sepsis will be important in directing management.
