Activities of daily living and related factors in patients with severe COPD

Introduction: With the progression of the disease and the experience of symptoms such as dyspnea and fatigue, the limitation to performing activities of daily living (ADL) becomes more evident. In this study, we aimed to investigate ADL levels and related factors in severe COPD patients. Methods: Seventy severe COPD patients participated in the study. Physical and sociodemographic data were recorded. Respiratory muscle strength, Six-minute walk test (6MWT), London Chest Activities of Daily Living Scale (LCADL), International Physical Activity Questionnaire-Short Form (IPAQ-SF), Saint George Respiratory Questionnaire (SGRQ), COPD Assessment Test (CAT), and Hospital Anxiety and Depression Scale (HADS) were used for all participants. For determining the variables that have the greatest influence on ADL, a multiple linear regression analysis was used. Results: The 6MWT distance, IPAQ-SF, and HADS were significant and independent determinants of the LCADL, with 31.3% variance in subjects with severe COPD. LCADL scores were positively correlated with CAT, SGRQ symptom, SGRQ activity, and SGRQ total; negatively correlated with FVC (%), FEV1 (pred%), and MIP (p˂0.05), but showed no significant correlation with, FEV1/FVC, MEP, and SGRQ-impact (p˃0.05). Conclusion: In this study, our sample of severe COPD patients had low levels of functional status in ADL, with the greatest influence on functional capacity, physical activity, anxiety, and depression levels. It is seen that ADL in severe COPD patients is affected by both physical and psychosocial factors.
