A retrospective study of incidence of COVID-19 associated pulmonary aspergillosis who are admitted to different department
Background: The incidence of CAPA reported 4%-30%,the extreme variation in reported incidence may attribute to several reasons. We conducted a study to understand the possible reason in different department. Methods: Between 1. Dec 2022-30. Sep 2023,A total of 871 COVID-19 patients were enrolled. Among them, 109 cases were initially admitted to ICU or EICU(non-RICU), 98 cases were in RICU, 238 in respiratory general wards, 426 in other departments. When disease worsened, patients transfer to ICU for management. Finally, A total of 175 cases have been admitted to non-respiratory ICU, 359 are in respiratory wards, of which 162 are in RICU,337 are in other departments. Results: 91 cases of CAPA were confirmed, incidence of CAPA in non-critical COVID-19 patients is 37/534 (6.9%), in critical COVID-19 patients who admitted in RICU is 30.8% (50/162), 6.8% incidence in non-RICU department (12/175). The average time from hospitalization to confirm CAPA was 6.26 days in 91 cases. 5.6 days among non-critical COVID-19 in respiratory general ward; 7.4 days among critical COVID-19 in non-respiratory ICU ;3.7 days in RICU after they admitted RICU. Number of patients sent for microbiological examination and evidence for confirmed CAPA show in figure. <fig><object-id>erj;64/suppl_68/PA3282/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> Conclusion: BALF sample is important for early diagnosis of CAPA, RICU is most appropriate for management of critical CAPA.
