Heterogeneity of comorbidities captured by European national severe asthma registries in SHARP CRC

Introduction: There is growing awareness that comorbidities and multimorbidity impact disease control and quality of life in patients with severe asthma. However, it remains unclear as to how well these conditions are recognised in clinical practice. National severe asthma registries benchmark clinical practice and offer an opportunity to assess how well comorbidities are being considered. Aim: We assessed comorbidities collected in 20 European severe asthma registries by studying the Federated Analysis Platform (FAP) of SHARP. Methods: The SHARP FAP was used to map comorbidities from 8 European national severe asthma registries in addition to SHARP Central – a harmonised data registry used by 12 countries. Results: 48 comorbidities were mapped across the 20 registries. T2-related conditions were best captured with chronic sinusitis and nasal polyposis included in 100% registries, allergic rhinitis 95% and eczema 90%. Several pulmonary (COPD 90%, bronchiectasis 85%, obstructive sleep apnea 75%), extrapulmonary (gastroesophageal reflux 100%, cardiovascular disease 90%) and psychophysiological (depression 85%) were also frequently recorded. Osteoporosis was collected in 85% but other steroid induced pathologies were less well recorded (cataract 70%, diabetes 55%, adrenal insufficiency 40% and glaucoma 25%). Conclusions: There is much variability in comorbidity data included in European severe asthma registries. T2 and pulmonary comorbidities are covered well, however the capture of potential steroid induced pathologies is suboptimal, revealing a possible gap in our daily clinical practice which could be improved with harmonisation of the registries in the near future.
