Thoracic involvement during Behçet's disease: about 32 cases
Introduction: Behçet's disease is a systemic disorder classified as a primary non-necrotizing vasculitis. Thoracic involvement, although less frequent than other manifestations, presents particular clinical and therapeutic challenges. Methods: We present a retrospective study of 32 patients from January 2002 to June 2023 at the respiratory diseases department of CHU Ibn Rochd, Casablanca. Its aim was to determine the clinical, therapeutic and prognostic features of thoracic involvement associated with Behçet's disease. Results: The thoracic manifestations in our patients were: thrombosis of the superior vena cava (13 cases), pulmonary arterial aneurysm (10 cases), pleural involvement (7 cases) including a chylothorax, cardiac involvement (6 cases) and thrombosis of the pulmonary artery (7 cases). The treatment offered to our patients was based on immunosuppressive and anticoagulant therapy, as well as colchicine in patients with mucocutaneous and/or articular lesions. The course of the disease was highly variable. It was fatal in six patients, mainly due to lightning hemoptysis in those with pulmonary arterial aneurysms. Conclusion: According to our study, the thoracic involvement associated with Behçet's disease, although rare, is serious and can compromise patient survival. Rapid diagnosis and immediate management are therefore essential.
