Surgical Management of Inflammatory Myofibroblastic Tumour of the Lung: A Comprehensive Case Series and Long-Term Outcomes Analysis
Background: Inflammatory myofibroblastic tumour (IMT) comprises 0.04–0.18% of all lung lesions Aims: Comprehensive case series focusing on surgical outcomes of lung IMT Methods: 20 patients had 21 surgical interventions (1998-2023). Preoperative work-up included complete evaluation according to guidelines. Follow-up was complete in all patients (range: 1 mo-25 ys) Results: We included 8 men and 12 women, 16 (76.1%) were asymptomatic at diagnosis. Preoperative IMT diagnosis was made in 17 (81.0%), although all had undergone transbronchial biopsy or transthoracic needle biopsy for preoperative diagnosis. Extent of surgical excision was: 12 (57.1%) atypical segmentectomies, 2 (9.5%) typical segmentectomy, 3 (14.3%) lobectomy, 1 (4.8%) bilobectomy, 1 (4.8%) pneumonectomy, 1 (4.8%) chest wall resection, 1 (4.8%) right anterior mediastinotomy. Complete tumour resection was accomplished in all. 30/90-days postoperative mortality was nihil. There were 5 (23.8%) postoperative complications (1 prolonged air leak, 4 atrial fibrillation). There was 1 (4.8%) recurrence; all other patients are alive and without evidence of disease. Mean overall survival was 128 months <table-wrap><object-id>erj;64/suppl_68/PA1405/TB1</object-id><object-id>T1</object-id><object-id>TB1</object-id><table><colgroup><col></col><col></col></colgroup><tbody><tr><td>Age, mean (SD)</td><td>45.1 (17.9)</td></tr><tr><td>BMI, mean (SD)</td><td>24.1 (4.8)</td></tr><tr><td>FEV1%, mean (SD)</td><td>95.6 (13.7)</td></tr><tr><td>DLCO/VA%, mean (SD)</td><td>99.3 (27.4)</td></tr><tr><td>Size (mm), mean (SD)</td><td>27.5 (3.7)</td></tr><tr><td>Operative time (min), mean (SD)</td><td>132.6 (93.6)</td></tr><tr><td>Length of hospital stay, median (range)</td><td>4 (1–10)</td></tr></tbody></table></table-wrap> Conclusions: This large case series provides valuable insights into the successful surgical management of lung IMT, emphasising the importance of complete resection in achieving favourable long-term outcomes and survival rates for affected individuals.
