Bi-centric Experience with Cytoreductive surgery and hyperthermic intra-thoracic chemotherapy (HITHOC) for pleural malignancies
Background: Cytoreductive surgery (CRS) and HITHOC in the management of different malignant pleural disease is still under investigation. There are many signals that CRS+HITHOC prolong survival in selected patients with pleural malignancies (PM) Aim: to investigate the outcome of CRS & HITHOC in patient with PM Methods: A bi-center retrospective. Study variables: age, sex, primary tumor, malignant pleural effusion, post op complications, pre-post operative chemotherapy. Primary end point: survival after surgery. Secondary end point: morbidity, mortality & recurrence. Statistical Analysis and survival curve using Kaplan-Meier Method. Results: 33 patients, mean age 58.2, 23 M, 10 F. Malignant mesothelioma 25 pts, 8 patients with pleural metastases, 2 of Ewing’s sarcoma , 2 lung cancer, 2 stage IV thymoma and 2 osteosarcoma. HITHOC was performed for 1 hour using cisplatin and in 6 patients was combined with doxicurbin. 25 patients underwent CRS+HITHOC with VATS and other 7 patient has thoracotomy. Hospital mortality was zero. 2 (6%) pts died before 6m, 4 (3%) died before 12m, 8 (27%) died before 24m. Most common complication was anemia. The mean survival for the entire group based on Kaplan Meier curve was 56.6 m (Figure 1) For epithelioid mesothelioma was 62.2 m and for thymoma was 43 months. Conclusion: our study confirms that CRS & HITHOC prolong survival in selected patient with PM <fig><object-id>erj;64/suppl_68/PA1420/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
