How to perform Hybrid Uniportal Lung Biopsy in less than 15 minutes
Background: Several surgical techniques have been described for wedge resection in patients with solitary nodules or interstitial lung disorders all requiring hospitalization. Methods: We report our experience with a hybrid uniportal wedge resection of the lingula which is performed extracorporeal in outpatient settings or as a bedside procedure in patients who cannot tolerate single lung ventilation. We analyzed age, sex, complications, pre and postoperative diagnosis and hospital stay. Results: we describe 13 patients 8 M, 5 F with mean age 59.4 years. Indications were bilateral ILD in 8 patients, positive PET nodule(s) in 4 and recurrent chest infection after lung transplantation in 1 patient. Two patients with a FEV1 < 50% were operated under sedation, and 1 mechanically ventilated patient with respiratory failure was operated at bedside in ICU. Specimens ranged from 2x2 to 3.8x2.5 cm. The final diagnosis was UIP in 4 pts, emphysema in 2, NSIP, IPF, lymphoma, carcinoid, metastatic adenoca, sarcoidosis and smooth proliferative response in 1 patient respectively. There were no intraoperative complications or mortality, operative times ranged from 5 to 15 min and hospital disharge was 24-48 hours in 11 pts. Conclusion: Hybrid Uniportal lung biopsy is simple, effective, safe, and quick as a day-case or bedside surgery. <fig><object-id>erj;64/suppl_68/PA1409/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
