Bronchodilator response in a Pediatric population: Clinical and functional profile of the discordant group between GINA and ATS-ERS guidelines

Introduction: There is a discrepancy concerning the definition of a clinical significant Bronchodilator (BD) response (BDR) between GINA and ATS-ERS guidelines. Studies reporting the clinical and functional profile of the discordant group [ATS-ERS+ & GINA-] are inexistent. Objective: To assess the clinical and functional profile of the discordant group. Methods: The following data of pediatric’ patients who performed a spirometry were analyzed: cough, wheezing, familial atopy, passive smoking, sex, age, height, body mass index (BMI), FEV1 and FVC, FEV1/FVC. In case of an obstructive ventilatory impairment (OVI) a BD test was performed. BDRs (ie; absolute; %Initial; %Predicted) for FEV1 were calculated. Three groups were identified: Group1: ATS-ERS- & GINA-; Group2: discordant group, and Group3: ATS-ERS+ & GINA+. The ATS-ERS considered a clinical significant BDR threshold to be 10%Predicted, whereas GINA set it at 12%Predicted. Results: Among the 278 patients, 19.8% had an OVI.
