Central retinal artery occlusion-telediagnosis

A 60 year old male presented with sudden onset loss of vision in left eye on waking up in the morning. Vision was only hand movement close to face with RAPD in left eye. Posterior segment image taken with smart phone and 20 D lens [Fig. 1] showed posterior pole retinal whitening, box caring of vessels, arterial attenuation, optic disc edema with presence of cherry red spot at the fovea, suggestive of central retinal artery occlusion. Central retinal artery occlusion is an ocular emergency. This report highlights the ease with which the retina can be imaged using a smart phone and 20 D lens and the image could then be used for tele-confirmation of diagnosis if required even in emergency settings. This can be crucial in saving both the lives and vision. Despite the grim prognosis for CRAO, efforts to re¬store vision should be instituted, preferably within 4 hours of symptoms.[1]Figure 1: Posterior segment image taken with a smartphone and 20 D lens showing posterior pole retinal whitening, box caring of vessels, arterial attenuation, and optic disc edema with the presence of a cherry red spot at the fovea.
