Retratamento endodôntico de dente portador de lesão periapical extensa com presença de núcleo metálico

This study aims to present a clinical case of retreatment of a tooth with an extensive periapical lesion with the presence of a metal core. A 59-year-old patient was referred for dental treatment due to the need for retreatment of tooth 22. During clinical examination, the presence of a fistula was observed and the patient reported pain on vertical and horizontal percussion, absence of a periodontal pocket, and radiographic examination showed extensive periapical bone radiolucency, presence of filling material, as well as a fused metal core. After removal of the metal core with the aid of ultrasound, retreatment was performed with rotary instrumentation (Prodesign Logic RT). Sodium hypochlorite at 2.5% was used as the irrigating solution. The root canal system was obturated using the single cone technique associated with BIO-C Sealer cement. It is concluded that endodontic retreatment after removal of a fused metal core can be performed in a single session and determine the success of endodontic therapy.
