#224 EmeraldDOC – hitting the ground running

Introduction
Starting anew, especially when it involves relocating to a new country, can be a daunting experience. This is particularly true for the 500 International Medical Graduates (IMGs) who begin their practice in Ireland each year. These IMGs face the challenge of not only adapting to a new healthcare system but also integrating into a different culture and way of life. Unlike other healthcare-professionals, IMGs do not receive specific induction and support in the Irish Health Service, despite the fact that the practice of medicine is deeply influenced by the social, cultural, and organisational context in which it is delivered. EmeraldDOC helps IMGs integrate smoothly into Irish healthcare, both professionally and personally. The project includes two key phases: Development of an IMG–tailored induction program, ‘EMeraldSIM,’ focused on non–technical aspects of Irish healthcare; Creation of a website, EmeraldDOC.ie, providing information and guides on both clinical and lifestyle challenges faced by IMGs in Ireland.
Methods
A survey of 100 IMGs highlighted housing and lifestyle issues as significant challenges (83% and 86%). Additionally, 98.5% desired an induction program covering cultural and related topics. Focus groups were held with IMGs to determine relevant topics for the induction program, with collaboration from key national medical organisations. The project was successfully funded by the National Doctors Training and Planning Development Fund.
Results
The inaugural EMeraldSIM, held in July 2024 as part of the South/SouthWest Hospital Group IMG induction program, saw 64 IMGs participate, with overwhelmingly positive feedback. 95% agreed that the program would positively impact their clinical care and adaptation to Ireland, and 99% would recommend it to other IMGs.
Conclusion
Phase One successfully demonstrated the effectiveness of teaching non-technical skills through simulation and small-group learning, using a hub-and-spoke broadcasting model. This project is now planned for national expansion, making it a crucial step forward in supporting IMGs in Ireland.
