Current Trends In Aerobic And Anaerobic Antibiogram Resistance Pattern In Diabetic Foot Infections – A Prospective Study Done In A Tertiary Care Hospital, Tamilnadu.
Diabetes is now a global pandemic, the global prevalence of diabetes is estimated to increase from 8.8% in 2015 to 10.4% by the year 2040 .Much important have to be given for the identification and antibiogram resistance pattern of the anaerobic organisms isolated from the infected diabetic foot ulcers. The aim is to isolate and identify the bacterial aerobic and anaerobic antibiogram resistant patterns.Materials and Methods: A prospective cross sectional study was carried out over a period of 3 yrs.206 samples were collected.Tissue bits, biopsy, aspirated fluids and Amputated specimens were collected directly from the bed side and operation theatre for anaerobic culture.VITEK 2 system was used for identification of the isolated anaerobic organism, and for antibiotic susceptibility testing.RESULTS: Among the 206 Diabetic Foot Ulcer (DFU), Wagner's grading of diabetic foot ulcer of Grade 3, constituted a major proportion (35.43%).Most of the sample collected was Tissues (83.98%).Monomicrobial infections were 49.02% and 41.74% of polymicrobial infections.32 isolated were anaerobic organisms.Among them Peptostreptococcus.sppwas the common isolate among anaerobic culture.S.aureus, 60.6% were MRSA.30% ESBL and 10 % MBL producers among enterobacteriaceae.Anaerobic susceptibility testing result showed MDR pattern except for clostridium spp.DISCUSSION: The percentage of anaerobes isolated goes in consonance with the studies done in the same geographical area.Fusobacterium spp showed 100% resistance to clindamycin, and 33.3% resistance to moxifloxacin.Bacteroids spp showed 100% resistance to penicillin, 87.5% resistance to ampicillin,90% resistance to clindamycin and 60% resistance rate to moxifloxacin.CONCLUSION: Implementation of antibiotic stewardship program will help in achieving optimal clinical outcomes and adverse events, to reduce the cost of health care, and limit the emergence of AMR strains among anaerobic infections.
